- W4313056875 abstract "<b>Background:</b> Teicoplanin is commonly used in critically ill patients with pneumonia. However, limited evidence is available for optimal regimen in critically ill patients. <b>Objectives:</b> To evaluate the population pharmacokinetics and pharmacodynamics of teicoplanin after multiple-dose administration in critically ill patients <b>Methods:</b> Fifteen adult patients with pneumonia hospitalized in a medical intensive care unit were included in this study. All patients received teicoplanin 6 mg/kg every 12 hours (q12h) for three doses followed by at least one maintenance dose based on their creatinine clearance (CRCL). After collecting serial blood samples, plasma teicoplanin concentration-time data were analyzed by non-linear mixed effects modeling. Optimal dosing regimen was determined based on the probability of target attainment (PTA) at the trough concentration (Ctr) of > 10 mg/L for various dosing regimens through Monte Carlo simulations. <b>Results:</b> A 2-compartment model best described the concentration-time data. CRCL was the only covariate significantly associated with systemic clearance (CL) and peripheral volume of distribution (V2) (P < 0.05 for both): CL = 0.629 × (CRCL/64)0.656 and V2 = 55.7 × (CRCL/64)-0.665. Intercompartmental distribution clearance and central volume of distribution was 5.71 L/h and 4.79 L, respectively. The PTA to achieve Ctr > 10 mg/L was < 60% for the dosing regimen of 6 mg/kg q24h in patients with normal renal function; however, 12 mg/kg q24h regimen achieved the PTA ≥ 85%. <b>Conclusions:</b> The currently approved teicoplanin dose might not achieve adequate systemic exposures in critically ill patients. Alternative high-dose regimens should be considered for optimal treatment outcomes." @default.
