These include the conservation of protein, the mobilization of free fatty acids from adipose tissue and inhibition of glucose uptake by body tissues. Therefore, somatotropin may be a more appropriate name than growth hormone. Physicochemical characteristics have been studied extensively, and the molecular weight (approximately 21,500), the amino acid content (188 amino acids) and the structural sequence determined. Production rates of growth hormone have not been determined because techniques currently available do not permit such measurements. However, certain normal stimuli, such as exercise and deep sleep, and certain abnormal stimuli, such as insulin-induced hypoglycemia and arginine infusion, cause a significant increase in HGH concentrations in serum. Therefore, growth hormone deficient states can be identified by analyzing serums, which are obtained following the administration of these stimuli, in the radioimmunoassay for growth hormone. Growth hormone deficient states may result from organic lesions in the hypothalamic-pituitary area, secondary to emotional deprivation and for unexplained or idiopathic reasons. The same clinical state is occasionally seen in patients who have measurable growth hormone, but peripheral resistance to this. This type of hypopituitarism and some instances of idiopathic hypopituitarism are genetically determined. Human growth hormone is an effective therapeutic agent in patients with hypopituitarism, in some patients with hypoglycemia and in some intrauterine dwarfs if given in large or pharmacologic amounts. It is ineffective or of limited effectiveness in chondrodystrophies, many types of primordial dwarfism and in gonadal agenesis. Unfortunately, animal growth hormones are ineffective in humans, and monkey growth hormone is impractical to use because of the small yield from monkey pituitaries. Human growth hormone has not been synthesized, and no totally satisfactory substitute has been found. For these reasons, the supply of HGH is limited, and only a few of the many deserving hypopituitary patients have had the benefit of receiving HGH.
