- W4313220650 abstract "Introduction: The data supporting an appropriate duration of antibiotic treatment for ventilator-associated pneumonia (VAP) is limited. Current guidelines still recommend short-duration antibiotic therapy for non-fermenting gram-negative bacilli (NF-GNB) VAP which may be associated with a higher recurrence of pneumonia. In this meta-analysis, we aimed to address this knowledge gap and compare the outcomes of randomized controlled trials (RCTs) of short- versus prolonged-course antibiotic regimens for VAP. Methods: We searched the Cochrane Library, PubMed, and Embase databases from their inception till June 2022 (PROSPERO CRD42022340615). We included RCTs that compared the effectiveness of a “short” versus “prolonged” course of antibiotic treatment in patients with VAP. Our primary outcomes were 28-day mortality and recurrence of pneumonia. Data analysis was performed using RevMan 5.4. Risk ratios (RRs) and mean differences (MDs) were calculated as effect measures. Results: Our pooled analysis consisted of six RCTs. For 28-day mortality, no significant difference was found between the prolonged course and the short course (RR 1.15; 95% CI: 0.86-1.52). Administration of a short course of antibiotics increased the risk of recurrence of pneumonia (RR 1.34; 95% CI: 1.02-1.75); however, in sensitivity analyses, when including trials published as full-length articles, trials using 8- versus 15-day regimens only, and trials using the same antibiotics in both arms only, the results became non-significant. Secondary outcomes, such as clinical resolution, duration of intensive care unit (ICU) stay, duration of mechanical ventilation, mechanical ventilation free days, and mortality at the latest reported time revealed no significant difference between the two regimens. The short course was, however, associated with a greater number of antibiotic-free days (MD 4.02; 95% CI: 2.26-5.78). Conclusions: Our meta-analysis showed that a short course of antibiotics was associated with a higher recurrence of pneumonia but more antibiotic-free days with no difference in mortality risk as compared to a prolonged course. The decision about the length of a regimen should be based on close biological and clinical monitoring taking into account relevant patient factors and the resource setting." @default.
