- W4313311058 abstract "症例は64歳，男性．４日間持続する右側腹部痛があり近医受診した．血液検査にて炎症反応の上昇を認めたため精査目的に当院紹介となった．右側腹部に限局する圧痛と腹膜刺激徴候を認め，腹部造影CTで右側腹部腹壁直下に大網の限局的な脂肪織濃度上昇と渦巻き状の大網血管を認め大網捻転症を疑った．同日腹腔鏡下での緊急手術を施行した．右側腹部腹壁に壊死した大網の癒着を認めた．大網の末梢は横行結腸腹膜垂に癒着し，癒着部位と10cm中枢の大網を基点として大網が時計回りに900度回転し壊死していた．壊死大網を切除し手術は終了した．大網捻転症は稀な疾患であり，壊死部位によって疼痛箇所が異なることから術前に確定診断に至らないことが多い．腹腔鏡手術は容易に確定診断が得られ，また低侵襲手術が行える点で有用と考えられた．" @default.
