- W4313311273 abstract "症例は80歳の男性．発熱で近医を受診し，血液検査で炎症反応の著明な上昇を認めたため，精査加療目的に当院を紹介受診した．腹部CT（computed tomography）で回腸終末部の脂肪織濃度上昇とfree airを認め，上行結腸に多発する憩室を認めた．上行結腸憩室穿通の術前診断で緊急手術を行った．術中所見では回腸末端の腸間膜に膿瘍形成をきたしていた．憩室が発生していた上行結腸とともに膿瘍形成部を切除する方針として右半結腸切除術を行い，機能的端々吻合で再建を行った．切除標本を確認すると，回盲弁から約3cm口側の回腸に穿通部位を認めた．病理組織診断で回腸憩室穿通および回盲弁のlipomatous hypertrophy（LH）が指摘された．術後経過は良好で，術後14日目に退院となった．Meckel憩室以外の小腸憩室は稀な疾患である．また，本症例では回盲弁のLHが随伴しており，回腸末端の憩室の原因となった可能性が考えられた．" @default.
