Prinzipiell soll allen Tumorpatienten ein regelmäßiges Screening auf Ernährungsstörungen angeboten werden und bei auffälligem Screeningbefund eine sorgfältige Diagnostik zur Klärung der zugrunde liegenden Ursachen erfolgen. Im Fokus stehen die Nahrungsaufnahme und mögliche störende Beschwerden, der Gewichtsverlauf, die körperliche Leistungsfähigkeit, die Körpermuskelmasse und das Vorliegen einer systemischen Inflammationsreaktion. Da während einer Tumortherapie gastrointestinale Beschwerden die ausreichende Nahrungsaufnahme gefährden, sollte regelmäßig eine individuelle ernährungstherapeutische Begleitung angeboten werden. Nach kurativer Behandlung besteht das Risiko für ein metabolisches Syndrom, deshalb sind eine ausgewogene Kost und regelmäßige körperliche Aktivität zu empfehlen. Bei palliativen Therapien muss auf die Entwicklung einer Mangelernährung geachtet werden. Besonders bedrohlich ist das Bild der Kachexie mit der Kombination aus unzureichender Nahrungsaufnahme, Inaktivität und kataboler Stoffwechsellage. Die Behandlung einer Kachexie erfordert ein multiprofessionelles Vorgehen mit Sicherung der Nahrungszufuhr, Anleitung zum Muskeltraining und Ansätzen zur Normalisierung der Stoffwechsellage. Zusätzlich sollte einem Bedarf an psychologischer und sozialer Unterstützung entsprochen werden. Nahrungsergänzungsmittel spielen keine relevante Rolle; Mikronährstoffmangelzustände müssen jedoch ausgeglichen werden: Am Lebensende ist die Betreuung primär darauf gerichtet, belastende Symptome zu lindern. Um alle von Ernährungsstörungen betroffenen Patienten zuverlässig zu unterstützen, müssen feste Strukturen, Zuständigkeiten und standardisierte Abläufe etabliert werden." @default.
