The Journal policy requires editors and reviewers to disclose conflicts of interest and to decline handling or reviewing manuscripts for which they may have a conflict of interest. The editors and reviewers of this article have no conflicts of interest. We read with interest the article on the novel “Y” incision/rectangular patch aortic annulus enlargement technique by Professor Bo Yang,1Yang B. A novel simple technique to enlarge the aortic annulus by two valve sizes.J Thorac Cardiovasc Surg Tech. 2021; 5: 13-16Google Scholar which has been clearly described with great illustrations. We congratulate Professor Yang on such a great solution to small aortic roots. We recently performed our first “Y” incision/rectangular patch root enlargement in our unit. We found Professor Yang1Yang B. A novel simple technique to enlarge the aortic annulus by two valve sizes.J Thorac Cardiovasc Surg Tech. 2021; 5: 13-16Google Scholar right to describe the technique as a “simple” way to enlarge the aortic annulus. Before this letter, this technique had been described as the “Y” incision/rectangular patch technique to clearly describe the “Y” configuration of the incision carried down between the left coronary and noncoronary commissure post, as well as the rectangular shape of the patch used for the reconstruction of the enlarged root.1Yang B. A novel simple technique to enlarge the aortic annulus by two valve sizes.J Thorac Cardiovasc Surg Tech. 2021; 5: 13-16Google Scholar,2Yang B. Naeem A. However, we have also realized the procedure needs a name apart from the description of its 2 main technical components: the incision type and patch shape. After obtaining consent from Professor Yang via an email communication on our proposal to rename the procedure after him, we are happy to formally rename the procedure “The Yang Procedure” in honor of the inventor, Professor Bo Yang. Editorial team, allow us to introduce the “The Yang Procedure,” a posterior aortic annular enlargement technique with a “Y” incision and a rectangular patch as the 2 main components to enlarge the aortic annulus by 3 to 4 valve sizes as described by Professor Bo Yang. A novel simple technique to enlarge the aortic annulus by two valve sizesJTCVS TechniquesVol. 5PreviewThe current techniques of aortic root enlargement used by adult cardiac surgeons are the Nicks1 and Manouguian procedures.2 The Nicks procedure generally increases aortic annulus by one valve size.1 The Manouguian requires incising the mitral valve (MV) anterior leaflet and left atrium (LA), with risk of mitral regurgitation.3 Reported herein is a new surgical technique to enlarge the aortic annulus by 2 valve sizes without violating the LA or MV.
