- W4313443596 abstract "Despite an overall decrease in hospitalisation for acute coronary syndrome (ACS) in early 2000's, an increase in hospitalization for ACS among people aged under 65 years, and particularly among women has been observed. To analyse latest time trends in the rates of patients hospitalized for ACS in France by sex and age and determine the sex-differences in trends, care and outcomes from the French National Insurance Database, a quasi-exhaustive and nationwide database. All patients hospitalized for ACS in France between 2009 and 2019 were included. Rates of hospitalized ACS patients were standardized to the age of the 2010 European population and were computed for the overall population and separately by type of ACS (NSTSE-ACS, STSE-ACS) and by sex and age (< or > 65 years). Patients’characteristics were compared according to sex. Management, hospital mortality, early hospital mortality (in the first 3 days) and 90-day mortality rates and outcomes were adjusted on age, sex and comorbidities. In 2019, about 50,000 patients were hospitalized for STSE-ACS and 85,000 for NSTSE-ACS, corresponding to an age-standardized incidence rate of 71 and 123/100,000 person-years (PY) respectively. These incidences were decreasing since 2009 in all age and sex subgroups, except for STSE-ACS in men and women under the age of 65 (+5.3% in men; +14.4% in women, between 2009 and 2019). Over the last few years, the increase remained significant among middle-aged people, growing among women aged under 65 years (+5.8% between 2009 and 2014, vs. +8.2% between 2014 and 2019) but being more moderate among men under 65 years (+3.0%, vs. +2.2%). Regarding hospital management, in 2019, we found a lower use of angiography, percutaneous coronary intervention and coronary artery bypass graft in women than in men, regardless of the type of ACS, age, cardiovascular history, and cardiovascular risk factors. In-hospital mortality was 11.3% for STSE-ACS and 3.5% for NSTSE-ACS, with no difference by sex, whereas early hospital mortality (in the first 3 days) was higher in women than in men after adjustment. In recent years, the dynamics of ACS remain unfavourable in young adults in France and particularly in women, with a continued increase in the incidence of STSE-ACS. The evolution of cardiovascular risk factors in this population could explain this dynamic. In addition, there continued to be sex-differences in ACS management and early hospital mortality." @default.
