- W4313443653 abstract "Magnetic resonance imaging (MRI) has an increasingly important role in the management of patients, including those with cardiac devices. Despite the available guidelines, the management of these patients with cardiac devices remains complex in clinical practice. Hence, a specific procedure is mandatory in patients with defibrillators (ICD) before MRI. A team able of resuscitate and to use external defibrillators are required. The purpose of this work was to assess the feasibility of MRI in patients with cardiac devices, particularly with regard to compliance with safety procedures and cooperation between cardiologists and radiologists. Between June 2020 and May 2022, 187 consecutive patients with pacemakers or ICD who needed MRI have been included in a non-controlled, observational, single-center study. For each patient, 27 items were collected, among which the MRI compatibility of the device, the availability of a chest X-ray and the absolute contraindications. The detection and the stimulation thresholds, impedances of each lead, telemetry collections and longevity data were recorded. The procedures were different in pacemaker and ICD patients. Among the patients who underwent MRI, 50 (26.7%) had MRI-compatible device, 80 (42.8%) had non-compatible device but considered to be at low risk of cardiac complications (leads from differents manufacturers, chest X-ray not available) or having a significant risk of loss of chance in the absence of MRI (certified non-compatible MRI device and no alternative imaging methods). Finally, 57 patients (30.5%) had definitive contraindication (as abandoned or epicardic leads and very old and end-of-life devices). Few threshold changes were noted however frequent stimulation disturbances have been induced by MRI, especially during the diffusion sequences. One major event occurred in a patient with acute ischemic stroke who underwent MRI (non-MRI compatible pacemaker). This patient had ventricular tachycardia after MRI, while the interrogation of telemetries before admission was normal. Our registry has established the feasibility of MRI in 69.5% of our cohort of patients, using a detailed checklist to secure these examinations. One serious side effect have been documented.
