- W4313443995 abstract "Decreased kidney function is independently associated with cardiovascular mortality and morbidity. Although diabetic patients with acute myocardial infarction, coronary artery disease (CAD) burden associated with albuminuria and microalbuminuria remains poorly investigated. In a single-center study, all consecutive diabetic patients hospitalized for an acute MI in coronary care unit were included. They underwent coronary angiography, blood sample and urine sample for albuminuria assessment on admission. Coronary lesion complexity was prospectively assessed by SYNTAX score and pre-specified angiographic criteria. Patients were compared according to: 1/No μalbuminuria [Alb/créat (U) < 30 mg/g], 2/μalbuminuria [Alb/créat (U) = 30–300 mg/g] and 3/Albuminuria [μAlb/créat (U) > 300 mg/g]. In total, 156 patients were included, of whom 27 (17.3%) had albuminuria and 70 had μalbuminuria (37.8%) (Table 1). Median age and cardiovascular risk factors were similar across the 3 groups. When compared with patient with no μalbuminuria, patient with μalbuminuria and albuminuria had a higher prevalence of peripheral arterial disease (11%, 20% and 37% respectively, P = 0.016). The age of diabetes has a strong influence on albuminuria (3 [0–8], 10 [2–18] and 17 [7–27] respectively, P < 0.001). At coronary angiography, CAD burden, assessed by SYNTAX score, was much higher in elevated albuminuria groups (9 [5–16], 13 [8–20] and 20 [8–32] respectively, P = 0.006). There was a trend for more diffuse CAD (three vessel disease or left main) in patients with albuminuria (29%, 42% and 48% respectively, P = 0.117). Patients with high levels of albuminuria presented more chronic total occlusion lesions (CTO) (7%, 17% and 26%, respectively, P = 0.042). This prospective study in diabetic patients hospitalized for MI shows that μalbuminuria and albuminuria are associated with severe CAD burden, with a higher SYNTAX score. The long-term prognostic impact of μalbuminuria on CAD burden needs to be explored." @default.
