Die vorliegende Arbeit diskutiert wesentliche Neuerungen vor dem Hintergrund der Versorgungssituation in Deutschland. Die Leitlinie verdeutlicht die zunehmende Bedeutung multidisziplinärer Expertise in der Diagnostik und Therapie ventrikulärer Arrhythmien. Die Umsetzung der Empfehlungen setzt neben Grundfertigkeiten der Kardiologie spezielle Kenntnisse in kardialer Pathologie, Humangenetik, kardiovaskulärer Bildgebung (v. a. der kardialen MRT) und invasiver Elektrophysiologie voraus. Erstmals sind Empfehlungen zum Basic Life Support in die Leitlinie aufgenommen. Der Stellenwert einer genetischen Untersuchung und der kardialen MRT-Bildgebung wurde deutlich aufgewertet. Die ICD-Therapie bleibt wesentlicher Bestandteil der Primär- und Sekundärprophylaxe des plötzlichen Herztodes, wenngleich die Empfehlung zur primärprophylaktischen ICD-Therapie für Patienten mit dilatativer Kardiomyopathie abgewertet wurde und stattdessen eine individualisierte Risikoeinstufung vorgeschlagen wird. So hat sich das Spektrum der Risikomarker neben der LV-Funktion zunehmend erweitert. Risikoscores erlangen eine größere Bedeutung. Die Katheterablation als interventionelle antiarrhythmische Therapie wird ebenfalls deutlich aufgewertet, insbesondere bei idiopathischen ventrikulären Arrhythmien wird sie überwiegend zur Erstlinientherapie. Die Katheterablation erhält überdies Vorrang gegenüber einer medikamentösen antiarrhythmischen Therapie bei KHK-Patienten mit rezidivierenden Tachykardien unter chronischer Amiodaron-Therapie. Für viele andere Patienten ist der ideale Zeitpunkt für eine Ablationstherapie weniger klar definiert." @default.
