- W4313650855 abstract "The restoration of proximal humeral anatomy in anatomic total shoulder arthroplasty (TSA) is paramount for clinical outcome and longevity of the prosthesis. The aim of our study was to determine whether there is a difference in the accuracy of restoration of proximal humeral anatomy between stemless and stemmed TSA versions of the Shoulder Modular Replacement system (Shoulder Modular System, LimaCorporate, San Daniele del Friuli, Italy). We retrospectively reviewed all patients who received an anatomic TSA for glenohumeral osteoarthritis in North Shore Hospital, Auckland, New Zealand between 2015 and 2018. Three independent fellowship-trained shoulder surgeons compared preoperative and postoperative proximal humerus anatomy using 6 parameters measured on plain radiographs: humeral head height, humeral medial offset, humeral head centering, humeral head diameter, neck shaft angle, and lateral humeral offset. We assessed the accuracy of restoration of proximal humerus anatomy with the anatomic reconstruction index (ARI), a scoring scale that ranges 0-10, a higher score indicating a more accurate reconstruction. We investigated a total of 63 shoulders, 43 (68%) in female patients and 20 (32%) in male patients, average age at surgery 70 years (range 44-86 years). No statistically significant differences were found in the 6 radiographic parameters measured postoperatively between groups. The average ARI was 7.29 in the stemless and 7.41 in the stemmed group (P = .742). The average ARI corresponded with a good reconstruction in both stemmed and stemless TSA. No statistically significant difference was observed in the distribution of very good/excellent, good, and poor reconstruction categories between groups (P = .660). We concluded that there is no significant difference in the accuracy of restoration of proximal humerus anatomy between stemless and stemmed TSA. The results indicate that both stemmed and stemless TSA can reproduce the proximal humerus anatomy with good radiographic results." @default.
