- W4318422370 abstract "When most people think about food allergies, culprits like milk, egg, peanut, tree nuts, fish, and shellfish come to mind. Rarely does one consider wheat, rice, or oat, as trigger foods. In fact, when wheat allergy is mentioned most clinicians and lay people are surprised to learn that anaphylaxis is possible and assume celiac disease or gluten sensitivity. While other grains are uncommon, IgE mediated wheat allergy may affect up to 1% of US children. Patients with wheat allergy (or other grain allergies) present with immediate symptoms of an allergic reaction (hives, angioedema, breathing concerns, and vomiting). Similar to others with IgE mediated FA, patients must avoid the food, carry an epinephrine auto-injector, and upon accidental ingestion are at risk for anaphylaxis and even death. Due to the reaction, patients are often referred to allergists for evaluation and allergy testing by skin testing or immunocap testing is positive (indicative of an IgE mediated mechanism). In addition, these patients are at risk for other forms of atopy (eczema, allergic rhinitis, and asthma) and additional IgE mediated food allergies. Patients may also outgrow their food allergy over time and are seen annually for re-evaluation and potential food challenges. Alternatively, in patients with celiac disease, gluten (wheat, rye, and barley) is in the diet at diagnosis and symptoms include abdominal pain, diarrhea, bloating, and in children, poor growth. Patients are often unaware of the cause of their symptoms and are ingesting gluten regularly. Once diagnosed, they are at increased risk of having or developing other autoimmune disorders and cancer. Diagnosis is made by serology and biopsy of the small intestine. Patients are followed by gastroenterologists and life-long avoidance is recommended. Patients with gluten sensitivity also have gluten in their diet at diagnosis and present with abdominal symptoms which improves with removal. While IgE mediated reactions to wheat are most common, other grains have been implicated as well. This article serves as a review of what is known about IgE mediated reactions to wheat, and albeit rare, other grains." @default.
