- W4319934985 abstract "Gastric Cancer (GC) belongs to the malignant diseases with one of the highest mortality rates despite declining incidences worldwide. The rapid advance of gene sequencing methodologies proposed at least four consensus molecular subtypes (CMS) which can be in part be reconciled with the traditional histomorphological classifications in individual patients, revealing causal relationships to oncogenic driver mutations and infection by pathogens. Therein, Helicobacter pylori (HP) and Epstein–Barr Virus (EBV) promote gastric carcinogenesis via defined virulence factors. In this context, both oral and intestinal microbiota have been suggested to compete for the ecological niche exposed by an HP colonized stomach. However, the contribution of non-HP microbiota and their metabolites, including the associated viromes and fungomes, to the development of GC remains largely elusive. The identification of distinct bacteria species and their pathogenic modes of action on mucosal epithelia and the host immune response shall open new avenues for prevention, early diagnosis and therapy of GC. In this book chapter, we will summarize the current state of knowledge on the gastric microbiome with respect to the transition of inflammation to pre-malignant states and progression to adenocarcinoma. An overview on specific pathogenic mechanisms and potential modalities for microbiota intervention treatments will be given, with a focus on clinical therapies with immune checkpoint inhibitors in patients." @default.
