Dadurch wurde klar, dass eine lokale Bestrahlung neben der Induktion von Tumorzelltod auch zur Auslösung und Modulation von lokalen und systemischen antitumoralen Immunreaktionen beiträgt. Um eine möglichst effiziente und lang anhaltende antitumorale Immunität zu erzeugen, müssen die immununterdrückenden Effekte der Strahlentherapie durch Immuntherapien blockiert oder die immunaktivierenden Effekte noch weiter verstärkt werden. Die zugrundeliegenden immunbiologischen Mechanismen werden in diesem Kapitel im Detail erklärt und diskutiert. Es gilt dabei immer, die unterschiedlichen und sehr individuellen Interaktionen zwischen dem Immunsystem und dem Tumor zu berücksichtigen. Dafür müssen geeignete Biomarker wie Tumormutationslast und Immunmatrices definiert werden. Die klinische Evidenz für den abskopalen Effekt der Strahlentherapie wird an Fallberichten erläutert und die Radioimmunotherapie als neues multimodales Therapiekonzept mit Fokus auf Immuncheckpoint-Inhibitoren anhand klinischer Studien vorgestellt. Es zeigt sich, dass die Radioimmunotherapie auch bei zerebralen Metastasen wirksam sein kann und die Toxizitäten beherrschbar sind. Radioimmunotherapien sind eine der vielversprechendsten Behandlungsoptionen für Patienten mit metastasierten Tumorerkrankungen, jedoch gilt es, in Zukunft noch detaillierter herauszufinden, welche Bestrahlungsprotokolle (Dosierung, Fraktionierung, Zielvolumina) und zeitliche Abfolgen von Strahlentherapie und Immuntherapien am effektivsten für bestimmte Patientengruppen sind." @default.
