- W4320341954 abstract "Cervical spondyloptosis is the most severe form cervical spine injury where patients usually develop disabling neurological deficits. The absence of significant neurological symptomatology is rare and most of the cases that have been reported previously show fracture or injuries to the components of the posterior arches allowing them to develop a traumatic decompression. In our bibliographic research within the English written literature, we were able to identify only four cases who developed traumatic spondyloptosis without posterior arch fractures and evidenced minimal neurological manifestations. We herein describe the management of a 34-year-old man who suffered a severe rollover car accident developing severe neck pain and lower limbs proprioceptive symptoms manifesting with walking disturbances secondary to a C6-C7 spondyloptosis. Despite having minimal progressive neurological symptomatology, it is important to emphasize that the elements of the posterior arch were preserved in this patient. To our knowledge this is the fifth case reported in English written literature with these characteristics. We review these cases and analyze some of the literature concerning the mechanism and options that may explain this behavior. Facing any traumatic event with considerable aggressiveness, every patient who report severe cervical pain should be carefully screened. Persistence of sagittal canal diameter is considered as the most determinant factor for neurological preservation in these injuries. The goals in the treatment of any cervical spine injury should include realignment of the spine with maintenance of spinal stability, prevention of further neurological insult, improvement of neurological recovery and promotion of early functional recovery. Surgical experience and proper hardware layout are essential to develop adequate surgical options. Cervical spondyloptosis without neurological deficit maintain a good prognosis despite being spectacular spinal injuries but their management must be multidisciplinary and properly instituted." @default.
