• W4321503186 abstract "Die akute Distorsion des oberen Sprunggelenkes zählt zu den häufigsten Sportverletzungen / Bewegungsverletzungen, die häufig in eine chronische Gelenkinstabilität mit langanhaltenden Folgebeschwerden übergeht. In der vorliegenden Follow-up-Studie wurden der 3-monatige ambulante posttraumatische Symptomverlauf, die Dauer der Arbeitsunfähigkeit sowie die Patientenzufriedenheit in einem gemischten Patientenkollektiv, welches primär ambulant in einer Notaufnahme versorgt wurde, erfasst. Zwei (NU1), sechs (NU2) und 12 Wochen (NU3) nach einem akuten Supinationstrauma am Sprunggelenk erfolgte eine monozentrische Nachuntersuchung der Patienten über einen Studienzeitraum von 13 Monaten. Neben subjektiven Beschwerden wie Instabilitätsgefühl und Schmerzintensität umfasste die klinische Untersuchung den Druckschmerz des lateralen Bandapparates und der vorderen Syndesmose, das Vorhandsein von Hämatom und Schwellung sowie Instabilitätstests als objektive Parameter. Einzelne Befunde wurden prognostisch hinsichtlich ihrer Relevanz auf Outcomeparameter wie Arbeitsunfähigkeit oder weiter bestehendem Instabilitätsgefühl statistisch untersucht. Das durchschnittliche Alter der 57 eingeschlossenen Patienten betrug 35 Jahre. Im Verlauf der Untersuchungszeitpunkte NU1 (n = 57; 100%), NU2 (n = 46; 80,7%) und NU3 (n = 26; 45,6%) reduzierten sich der Druckschmerz (p = 0,003) sowie die Schwellung (p = 0,001) signifikant. Ein Instabilitätsgefühl wurde zur NU2 von 87,7% der Patienten und zur NU3 von 57,7% angegeben. Dieses korrelierte bei 37% der Patienten mit dem Schubladenphänomen und bei 6,6% mit der lateralen Aufklappbarkeit (resp. 38,5%/ 11,5% zu NU3) und war signifikant mit dem Druckschmerz am LFTP und LFC zu NU2 und LFTC zu NU3 assoziiert. 70,2% der Patienten erhielten eine Arbeitsunfähigkeitsbescheinigung. Eine Orthese wurde zum Zeitpunkt NU2 von 40 Patienten (83,3%) und NU3 von 23 Patienten (46,9%) leitliniengerecht getragen. Die hohe Rate an persistierenden Beschwerden, die mangelnde Berücksichtigung initialer Therapieempfehlungen und die unzureichende qualitative und quantitative ärztliche Nachsorge erfordern ein verbessertes posttraumatisches Management nach OSG-Distorsionen. Die leitliniengerechte Orthesenbehandlung sowie das propriozeptive Training stellen den Grundpfeiler der Therapie. Die Schulung der Nachbehandler hinsichtlich pathognomonischer Symptome einer chronischen Instabilität oder weiterer Begleitverletzungen scheint darüber hinaus notwendig. The acute ankle sprain is one of the most common sports injuries which often turns into chronic joint instability with long-term secondary symptoms. In the present follow-up study over 3 months posttraumaticly, the duration of the inability to work and the patient satisfaction were recorded in a mixed patient collective, which was primarily treated on an outpatient basis in an emergency room. Two (NU1), six (NU2) and 12 weeks (NU3) after an acute supination trauma to the ankle a single-center follow-up examination of the patients was carried out over a study period of 13 months. The following parameters were examined: post-traumatic clinical course of complaints over 3 months, duration of incapacity for work and patient satisfaction. The mean age of the 57 patients was 35 years. In the course of the examination times NU1 (n = 57; 100%), NU2 (n = 46; 80.7%) and NU3 (n = 26; 45.6%) the pressure pain (p = 0.003) and the swelling (p = 0.001) increased significantly. A feeling of instability was reported at NU2 by 87.7% of the patients and at NU3 by 57.7%. This correlated in 37% of the patients with the drawer phenomenon and in 6.6% with the lateral opening (responding to 38.5%/ 11.5% at NU3) and was significantly related to the tenderness on the LFTP and LFC at NU2 and LFTC NU3 associated. 70.2% of the patients received a certificate of incapacity for work. The orthosis was worn by 40 patients (83.3%) at NU2 and 23 patients (46.9%) at NU3 onwards. The high rate of persistent complaints, the lack of consideration of initial therapy recommendations and the inadequate qualitative and quantitative medical follow-up care require improved post-traumatic management after ankle sprains. The guideline-compliant orthotic treatment and proprioceptive training are the cornerstones of therapy. Training of follow-up therapists with regard to pathognomonic symptoms of chronic instability or other accompanying injuries also seems necessary." @default.
