- W4324137090 abstract "重症病態下において特徴的に認められる負の骨格筋蛋白バランスは長期間にわたり継続的に認められる. 重症病態下では各臓器にて蛋白合成に必要な基質となるアミノ酸は骨格筋蛋白分解により供給されることから骨格筋蛋白分解が亢進し, その結果としてlean body mass（LBM）の喪失がおこり臨床的アウトカムが悪化する.LBM喪失を防止するために蛋白・アミノ酸投与は重要な手段となりうるが重症病態では遊離アミノ酸が十分に投与されても蛋白異化状態が改善されないいわゆる‘anabolic resistance（同化抵抗性）’となり, 蛋白合成速度と分解速度とのバランスが崩れた状態が継続し蛋白異化状態から離脱することが困難となる.したがって‘anabolic resistance’の状態にある患者に対していかに効率的に蛋白・アミノ酸代謝を改善するかが患者のアウトカム改善に最も重要なポイントとなる.現在, 重症病態患者を対象にLBM喪失を防止することを目的とした蛋白・アミノ酸投与が行われているが, 投与すべき蛋白・アミノ酸の種類・至適投与量に関しては今でも議論が継続している.これらの患者に特徴的に認められるLBM喪失による負の骨格筋蛋白バランスの機序について十分に考慮に入れたうえで患者の病態に応じた適切な栄養管理・治療が望まれる." @default.
