She had no worrisome historical features; on examination, she had normal vital signs and lumbar back and left lower quadrant abdominal tenderness. Pelvic ultrasound revealed a left adnexal mass measuring 7.1×5.5×6.5 cm with multiple echogenic and shadowing foci corresponding to the teeth visualized on the lumbar radiograph. She was discharged home and later underwent a laparoscopic left ovarian cystectomy with complete resolution of the symptoms. Dermoid cysts (mature teratomas) are one of the most common benign ovarian tumors and contain structures derived from all 3 germ cell layers.
