The main focus of the study was to report our initial experiences with this procedure and to describe the technique with some tips and tricks, but not on the reduction in the rate of GERD. Thus, we defined the primary outcome as the successful completion of all the steps of POEM-F. Future studies with longer follow-up times should define the primary outcome as clinical success and evaluate long-term outcomes, including GERD-related parameters. We agree with Ullah et al1Ullah S. Nonetheless, the ideal patient who would benefit from POEM-F is still being determined. During the early stages of this novel procedure, the ideal patient is younger and treatment naïve without advanced achalasia or a prior history of upper GI surgery. We are excited by the readers' interest in our study, and we look forward to further exploring the role of POEM-F in the management of achalasia.
