The indicators of all causes of death, from circulatory diseases, coronary heart disease, cerebrovascular diseases and stroke in the territory of the Tomsk region, profile of hospitalization, structure of acute cerebrovascular accidents were analyzed for several years. Particular attention was paid to case fatality rate, one of the key indicators of the effectiveness of the system of care for patients with stroke.Typical for many regions of the Russian Federation predominance of chronic forms in the structure of mortality from circulatory diseases, the absence of significant differences in the structure of strokes, as well as the features of the Tomsk region in the form of high levels of hospitalization profile and hospital case fatality rate, were revealed. The dependence of hospital mortality on logistics is shown, on the basis of which assumptions are made about the possible causes of high fatality rates in the region: excessive centralization of the system of vascular centers and the absence of really working mechanisms for timely reevacuation from them.To bring chronic circulatory diseases structure in line with international standards, it is necessary to regulate the rules for formulating and coding diagnoses. In order to reduce hospital fatality rates in the Tomsk region, it is necessary to carry out organizational measures: opening a primary vascular department in the area of responsibility of the regional stroke center, as well as strengthening rehabilitation and palliative services for the timely reevacuation of patients from vascular centers.Изучить динамику популяционных показателей: общего числа умерших от болезней системы кровообращения (БСК), ишемической болезни сердца (ИБС), цереброваскулярных болезней (ЦВБ) и инсультов, профильность госпитализации при инсультах, их структуру и летальность от них на территории Томской области в течение нескольких лет в сравнении с этими показателями по РФ и Сибирскому федеральному округу (СФО).Ретроспективное исследование было проведено на основании данных мониторинга острых нарушений мозгового кровообращения (ОНМК) и Территориального органа государственной статистики по Томской области в сравнении с данными литературы. Проанализированы за несколько лет показатели умерших от всех причин, от БСК, ИБС, ЦВБ и инсульта на территории Томской области, профильность госпитализации, структура ОНМК. Особое внимание было уделено одному из ключевых индикаторов эффективности системы оказания помощи пациентам с ОНМК — летальности.Выявлено характерное для многих регионов РФ преобладание в структуре смертности от БСК хронических форм, отсутствие существенных различий в структуре инсультов, а также особенности Томской области в виде высоких уровней профильности госпитализации и госпитальной летальности. Показана зависимость госпитальной летальности от логистики, на основании чего сделаны предположения о возможных причинах высоких показателей летальности в регионе: излишняя централизация системы сосудистых центров и отсутствие реально работающих механизмов своевременной реэвакуации из них.Для приведения структуры хронических БСК в соответствие с международными нормами необходимо регламентировать правила формулировки и кодирования диагнозов. С целью снижения госпитальной летальности на территории Томской области необходимо проведение организационных мероприятий: открытие в зоне ответственности регионального сосудистого центра первичного сосудистого отделения, а также укрепление реабилитационной и паллиативной служб для своевременной реэвакуации пациентов из сосудистых центров." @default.
