- W4360919333 abstract "Fluid therapy is a cornerstone of treatment to prevent or attenuate acute kidney injury (AKI) in acutely ill patients, but inappropriate fluid administration can lead to delayed recovery, fluid accumulation, and organ dysfunction. Both, hypo- and hypervolaemia can contribute to the development and progression of AKI. The prescription of fluid should be individualized by consideration of patient characteristics, volume status, phases of fluid therapy, acute and chronic comorbidities, and the nature and trajectories of the acute illness. Crystalloid fluids are recommended as first-line therapy, but recent evidence provides conflicting results regarding the type of crystalloid and kidney function. Certain types of colloid are harmful, and albumin has a limited role in the prevention of AKI and need for renal replacement therapy (RRT). A conservative fluid regimen is safe compared with a liberal fluid regimen in patients with acute respiratory distress syndrome (ARDS), septic shock after initial resuscitation, and in AKI. Fluid therapy should be discontinued once organ perfusion improves, patients are fluid-nonresponsive, or if they develop fluid accumulation. De-resuscitation should be considered to prevent and alleviate fluid accumulation syndrome. In patients treated with RRT, the same principles of fluid therapy and monitoring apply, with careful adjustment of ultrafiltration rates to correct fluid accumulation, prevent haemodynamic instability, and avoid organ hypoperfusion." @default.
