- W4363676405 abstract "Combining brain stimulation with brain imaging is a natural pairing that can greatly enhance knowledge of how stimulation engages the brain and how this engagement contributes to therapeutic outcomes. Image guided brain stimulation is a cutting-edge approach showing great promise in improving symptoms. However, these studies do not typically include measures of how brain stimulation engages neural circuits at the individual level or how the brain changes in response to the intervention. Our group is working to determine optimal strategies for using MRI to guide transcranial magnetic stimulation (TMS) for engaging specific neural pathways measured with interleaved TMS/fMRI. At the same time, we are treating patients with image-guided TMS and showing exciting preliminary results. To integrate these two lines of research, we have now established that pre-intervention circuit engagement, indexed by TMS/fMRI, is a prognostic predictor of symptom improvement in depression. In addition, we show associations between TMS-evoked fMRI brain response changes and therapeutic change. We interpret these findings as evidence that circuit integrity measured with TMS/fMRI is clinically valuable. Our results also provide mechanistic information on how neuromodulation affects the brain and contributes to symptom change. We contend that TMS is an effective tool for individualized neural circuit engagement and modulation.
