The world continues to age rapidly. By 2050, one in six people worldwide will be over age 65. To meet the needs of older populations while controlling costs, health systems must harness the potential of PHC. Multimorbidity and rising chronic disease burdens among older adults demand better clinical management, including coordination between providers and integration of care teams. Strong PHC incorporates these features, but PHC systems in most countries remain under-resourced and underperforming. Age-responsive PHC will need to extend beyond traditional clinical facilities to encompass home-based care and institutional care settings, with improved systemwide connectivity. The preferences of older people and their families can inform new, PHC-led care models, starting from a robust assessment of current system assets and deficits.
