- W4366088049 abstract "Psychiatric comorbidities (PCs) such as mood or anxiety disorders are associated with the use of isotretinoin, a highly effective treatment for acne vulgaris, but data on demographic disparities related to PC development is limited. We conducted a retrospective cohort study using the “All of Us” national American database and calculated odds ratios to assess if race, ethnicity, and age were risk factors for PC development, including mood, anxiety, psychotic and eating disorder as well as insomnia or attention-deficit/hyperactivity disorder. Out of the 327,654 patients in the database, 545 were prescribed isotretinoin. Of the 545, 134 (25%) had at least one newly diagnosed PC within 3-years of taking isotretinoin. The most predominant PC was mood disorder, occurring in 98 patients. Regarding race, we included Caucasians and African-Americans. Only Caucasians had increased odds of any new-onset PC within 3 years of taking isotretinoin (OR=2.4, P=0.0004). Of the psychiatric comorbidities, Caucasians specifically had increased odds of being diagnosed with anxiety (OR=2.3, P=0.004) and mood disorder (OR=2.7, P=0.0006). No significant relationships were found for Hispanic ethnicity. We included 3 age groups, ages 18 to 44, 45 to 64, and 65 years and older. Those 18 to 44 had decreased odds (OR=0.5, P=0.003) and those 65 and older had increased odds (OR=2.2, P=0.02) of developing at least one PC. Specifically, those 18 to 44 had lower odds of developing mood disorder (OR=0.4, P=0.0001), and those older than 65 had higher odds of developing insomnia (OR=3.1, P=0.02). Interestingly, despite not being significantly at risk for PCs when pooled together, those aged 45-64 had increased odds of developing mood disorder alone (OR=2.5, P=0.0003). We describe the prevalence (approximately one in four patients) of new-onset PCs following isotretinoin use, stratified by race, ethnicity, and age. There is a potential need to emphasize psychiatric support systems for patients. We also report unique risk profiles in different age and race groups that may represent underlying social mechanisms." @default.
