- W4366171462 abstract "Dermatologists prescribe the most outpatient oral antibiotics per provider, despite growing concerns for antimicrobial resistance. To our knowledge, there is no systematically designed or evidence-based antibiotic stewardship program commonly utilized in dermatology. This study aimed to identify potential antibiotic stewardship interventions and solicit feedback from key stakeholders using implementation science theoretical frameworks. We interviewed and surveyed 30 key stakeholders (22 dermatologists, 2 dermatology advanced practice providers, 2 dermatology residents, 4 infectious disease specialists) previously identified barriers to limiting use of oral antibiotics in acne to no longer than 3-4 months with topics grounded in the Theoretical Domains Framework. Identified salient domains to long-term antibiotic prescriptions were mapped onto the Capability, Opportunity, Motivation, and Behavior (COM-B) model Corresponding candidate behavioral change techniques were identified using the Behavioral Change Wheel. Stakeholders evaluated potential interventions according to affordability, practicability, effectiveness, acceptability, safety, and equity (APEASE) criteria. Interventions were included if 70% of stakeholders agreed all 6 APEASE criteria were met. Of 16 potential interventions, 5 met the APEASE criteria: prospective audit and feedback comparing one’s antibiotic prescribing rates to those of the specialty, individualized feedback on a random subset of antibiotic refills, electronic medical record prompts requiring justification for extended-course antibiotic refills, modeling sessions to practice counseling patients, and training sessions on prescribing oral contraceptives as antibiotic alternatives.
