- W4366173637 abstract "Shingles, known as zoster or herpes zoster, is a neurotropic viral disease featured with painful dermatomal skin eruptions, which leads to a notorious complication, post-herpetic neuralgia (PHN). PHN are usually refractory to treatment and disturbs patients' life. Among the world, stroke is a main cause of morbidity and mortality. Since patients are prone to get infectious diseases during acute phase following stroke, this study is aimed to investigate whether stroke increases the risk of shingles and it's terrible complication, PHN. Using the Taiwan National Health Insurance Research Database, this cohort study assessed patients who had shingles with and without a first incident of stroke. 20,551 post-stroke patients and 20,551 controls matched with age, gender, age categories and Charlson Comorbidity Index (CCI) score categories were enrolled. To evaluate the risk of shingles and its sequala in the post-stroke group relative to general population, Cox proportional-hazards regression models were employed. Within 5 years following stroke, the post-stroke group had a higher risk to develop shingles as compared with the control group (adjust HR = 3.82). Both ischemic-stroke group and hemorrhagic-stroke group were easily to get zoster infection (ischemic type (IRR = 2.51, 95% CI 2.09-3.02); hemorrhagic type (IRR = 2.31, 95% CI, 1.67-3.20)). Moreover, the ischemic-stroke group were prone to suffer from PHN (IRR = 2.24, 95% CI 1.56-3.20) whereas the hemorrhagic-stroke group tended to have herpes zoster ophthalmicus (IRR = 12.46, 95% CI 4.00-38.76). Since post-stroke people tend to develop shingles and PHN, physicians should be familiar with proper use of antiviral therapy and pain control to protect patients against herpes zoster and its sequala. Herpes zoster vaccine wound be recommended in post-stroke patients against the debilitating complication." @default.
