- W4366173670 abstract "While telemedicine is valuable in the care of patients with skin disease, its role in the management of Hidradenitis Suppurativa (HS) is uncertain. We sought to investigate the feasibility of longitudinal monitoring of disease activity from home in patients with HS. We recruited 27 patients with HS in a prospective observational study (October-December 2022). Participant demographics: 24 female, 9 Hispanic, 7 Black, 5 Asian, 4 non-Hispanic White, 2 other. Participants received automated weekly text reminders to upload photos of their HS affected skin (axillary, inframammary, inguinal or anogenital) and answer questions about pain and quality of life (QoL) (modified Pain VAS and Skindex Mini). Automated texts were sent from our Twilio based virtual assistant that included a link to a secure REDCap survey. Approximately 63% of participants completed at least 3 of 12 requests. The proportion of completed weekly uploads ranged from 23% to 63%. The total response rate was 16%; 40% after initial request, 39% after first reminder, 20% after second reminder and 3% after additional reminders. The average response time was 2 days. Over 12 weeks, we collected 275 photos. Two dermatologists reviewed photo quality (96% adequate distance, lighting and focus to identify clinical changes in nodules and overt drainage). 95% of photo uploads had complete QoL and pain scores. The average pain score was 3.6 out of 10 (SD 3.0). On Skindex scale of 0-6, the average quality of life was 3.1 Symptoms (SD 2.3), 3.4 Emotions (SD 2.3) and 2.8 Function (SD 2.3). We found that participants with axillary lesions were more likely to upload photos than participants with genital lesions (mean 4.3 uploads (axillary) vs 2.0 (genital)) suggesting that participant comfort in uploading photos may depend on body area affected. Barriers to photo upload included busy schedules, feelings of anxiety, depression, or embarrassment. Our findings suggest that longitudinal monitoring may be feasible in patients with HS, especially for axillary lesions." @default.
