- W4366178193 abstract "Systemic elevations of proinflammatory cytokines have been associated with a chronic inflammation state in patients affected with psoriasis. However, the impact of psoriasis on burn outcomes has not been described in the literature. This study evaluated the role of psoriasis on acute outcomes after burn injury. A retrospective cohort analysis was conducted using TriNetX, a federated health research network database. Population cohorts were stratified to compare patients with and without a prior diagnosis of psoriasis who sustained thermal or chemical burns. A 1:1 matched propensity score analysis (PSM) was conducted adjusting for demographics and percent total body surface area burned. Adjusted Risk Ratios (aRR) with 95% CI (p<0.05) were utilized to assess 30-day post-operative burn outcomes. Following PSM, balanced patient cohorts comprised of 7,093 patients per cohort were identified. Burn patients with preexisting psoriasis showed a significantly increased risk of infection of the skin and subcutaneous tissue (aRR [95% CI]) = (1.72 [1.56-1.90]), hyperglycemia (1.60 [1.25-2.07]), volume depletion (1.66 [1.35-2.04]), kidney failure (1.82 [1.54-2.16]), pulmonary edema (1.40 [1.51-2.92]), pneumonia (1.61 [1.36-1.90]), pulmonary collapse (1.35 [1.09-1.70]), acute myocardial infarction (1.61 [1.31-1.98]), acute embolism and thrombosis of the lower extremity (2.37 [1.95-2.88]), and sepsis (1.50 [1.19-1.90]). Preexisting psoriasis did not increase the risk for skin graft complications, ARDS, ICU admission, and mortality. These results stipulate that patients with a history of psoriasis are at increased risk for severe acute complications following burn injury. Thus, a history of this condition or concomitant diagnosis with the injury is warranted and further research to elucidate the mechanism for this relationship is needed." @default.
