- W4366178194 abstract "Solid organ transplant recipients (SOTRs) have up to a 100-fold greater risk of cutaneous squamous cell carcinoma (cSCC). There is limited knowledge of the relative outcomes of SOTRs with cSCC compared to immunocompetent (IC) individuals. A single institution database of cSCC cases diagnosed between 2010-2022 yielded 53 cases of invasive cSCC in SOTRs. We performed 3:1 case-matching using gender, age, tumor diameter and year treated with 145 IC cSCC cases. Statistical analysis included Chi-squared and Kaplan-Meier (significant p-value <0.05). There was no significant difference between the two cohorts by tumor location or stage (BWH and AJCC8). Almost half of SOTRs had a two-agent immunosuppressive regimen (47.2%). Kidney was the most commonly transplanted organ (62.3%). There was a significant association between smoking and SOTR status (p=0.001). The highest proportion of IC patients were prior smokers (44.4%), while the highest proportion of SOTRs were never smokers (60.4%). There was a significant association between tumor differentiation and SOTR status, such that SOTRs were more likely to have a poor tumor differentiation (25% vs 7%) (p=0.006). SOTRs were also more likely to experience local recurrence (7.6% vs 0.7%) (p=0.019) despite no difference in treatment. SOTRs had worse overall survival (OS) at 60 months (47.8%) compared to IC patients (92.2%) (p<0.0001). Significant differences in disease specific survival (DSS) were not detected, with only two SOTR and no IC disease specific deaths. SOTRs with invasive cSCC have worse OS with no detected difference in DSS, though the study was likely underpowered to detect a statistically significant difference in this rare outcome. SOTRs with cSCC are more likely to have poor tumor differentiation and local recurrence than their IC counterparts, suggesting there may be differences in the underlying tumor-specific and stromal-dependent drivers of disease." @default.
