- W4366181570 abstract "The aim of this study was to search for possible markers able of early predict the immune reactivation which occurs in psoriatic patients on treatment with biologics. In this perspective, the polyclonal free light chains (FLCs) of the immunoglobulins, which include κ and λ chains, could be of some help. Indeed, FLCs were significantly increased in patients with ongoing biological treatment and both κ and λ chains significantly correlated with duration of therapy. The overall study population included 45 patients affected by mild-to-severe psoriasis with either ongoing biological treatment or without any current systemic therapy. Peripheral blood samples were taken from all patients and 10 healthy subjects in order to determine FLCs by quantitative nephelometric assay whereas the detection of antinuclear antibodies (ANA) was executed through immunofluorescence. Patients with κ and λ FLCs above the normality range had a higher odds to be ANA+ respect to those with FLCs values within the normality range. Moreover, among patients with FLCs levels above the normality range on treatment for more than 12 months showed an even higher odds to produce ANA, Both κ and λ chains resulted significantly higher also in Pso patients after 16 weeks of an anti-TNF-α therapy confirming that their increase could be warning of immune reactivation in relation of duration therapy. Thus, increased FLCs levels would seem to be a valuable tool to predict the immunological fluctuations which occur in these patients during biological therapy and to monitor their clinical management" @default.
