- W4366184877 abstract "Multi-kinase inhibitors, which are molecular targeted drugs, frequently cause hand-foot skin reactions. These include lesions, such as erythema, blistering, and hyperkeratosis, on the palms and soles that may be painful and cause difficulty walking, negatively impacting quality of life in patients. Although topical steroids, moisturizers, and wound dressings have been used as treatments, their efficacy against hand-foot skin reactions is limited often leading to dose reduction or cessation of molecular target drugs. Recently, we reported an azole antifungal agent reduced the cytotoxicity of sorafenib on keratinocytes by drug repositioning. The present study examined the therapeutic effects and cytoprotection mechanisms of antifungal agents on hand-foot skin reaction induced by sorafenib, a multi-kinase inhibitor. In a clinical trial, topical application of ketoconazole cream, an azole antifungal drug, significantly improved hyperkeratosis and scores on a pain numerical rating scale in patients with sorafenib-induced hand-foot skin reaction, which was used to assess severity according to Blum's classification. Our in vitro analyses showed that azole antifungal agents inhibited sorafenib-induced apoptosis of cultured keratinocytes and releasing some inflammation related cytokines from cultured keratinocytes. These findings suggest topical application of azole antifungal agents is a promising therapeutic approach for hand-foot skin reaction." @default.
