This includes the possibility that anaesthetists might have adapted the dose of sevoflurane to accommodate patient factors, thus mitigating the effects of sevoflurane in vulnerable patients. We were interested that they found no significant relationship between age-adjusted median sevoflurane minimal alveolar concentration fraction or area under the sevoflurane dose curve on the incidence or severity of delirium. They also found no association of sevoflurane dose with predetermined delirium biomarkers. However, we note that the data analysed in this prospective cohort study were obtained from the ongoing Interventions for Postoperative Delirium: Biomarker-3 (IPOD-B3) study,2 which is designed to characterise the relationship between pre-morbid brain activity and postoperative delirium, not the relationship between sevoflurane exposure and delirium.
