• W4366979551 abstract "To determine factors associated with arthrofibrosis warranting early reoperation for lysis of adhesions following ACLR with soft tissue QT autograft. Specifically, we sought to determine how soft tissue QT graft size was associated with the incidence of arthrofibrosis. All primary QT autograft ACLRs within a single surgeon’s prospectively collected database were reviewed. Patients who underwent multiligament knee reconstruction or a cartilage restoration procedure were excluded. Graft size was calculated by averaging tibial and femoral tunnel diameter. Arthrofibrosis was defined as requiring a second procedure for lysis of adhesions due to inability to regain terminal extension within 6 months of ACLR. Sex-specific incidence of arthrofibrosis was evaluated relative to graft size. 1008 patients who underwent a total of 1054 QT ACLRs were included (44% female). Females had a higher incidence of arthrofibrosis compared to males (8.1% vs 3.4%, RR 2.4 [95% CI 1.4-4.0], p=0.0013). In each case, a cyclops lesion was present. Males with a graft size >9.0mm had an increased incidence of arthrofibrosis compared to graft size ≤9.0mm (6.3% vs 1.7%, RR 3.8, 95% CI:1.5-9.7, p=0.0028). Concomitant meniscal repair was associated with increased risk of secondary procedure for arthrofibrosis in females (17.9% vs. 6.8%, RR: 2.8 [95% CI 1.4-5.5], p=0.002). Graft size decreased among both males and females during the study period (2011-2015 vs 2016-2020), from 9.5±0.7 mm to 8.7±0.5 mm for males and from 9.4±0.7 mm to 8.6±0.4 mm for females (both p<0.001). This corresponded with a trend toward a reduced incidence of arthrofibrosis among males (5.2% vs. 2.3%, p=0.05) but no change among females. Quadriceps tendon autograft ≤ 9.0mm was associated with a decreased risk of arthrofibrosis among males. Females had a higher risk of arthrofibrosis than males that was not significantly associated with graft size but was associated with meniscal repair." @default.
