They did so with a national database, with reach to an impressive 87 million patient medical records. They found a general increase in vasectomy-naïve men undergoing vasectomy from 2019–2023, but specifically a significant 20% increase in vasectomies in the 7 months after the ruling compared with the 7 months prior. Limitations include the delay in reporting in the database, as well as its retrospective nature, but the investigators used incidence to try to alleviate some time lapse bias. Reproductive care in the United States has seen major shifts since the monumental Supreme Court abortion ruling on June 24, 2022. There is no doubt that men in the United States quickly realized the significant impact, as evidenced by the increase in internet searches and consults, leading to a transference of reproductive responsibility to them. At our institution, we have seen an uptick in vasectomy consult requests, although as this study suggests, there seemed to be an increase even in the past 3 years leading up to the reversal. Access to urologic care is an important aspect of male reproductive care, as vasectomy offers a safe, effective, and well-tolerated outpatient option. As the investigators note in this study, they do not show individual state or urban vs. rural trends, with the database only providing regional data. An interesting addition to these data would be an analysis of access and what the trend was for patients in urology-sparse areas, which may provide insight into improving underserved outreach. Nonetheless, this study adds significant weight to the literature regarding the importance of vasectomy services, especially in the wake of this political change.
