- W4367333486 abstract "There are many advantages of transforaminal endoscopic lumbar discectomy (TELD). Simply put, TELD can only resolve disc herniation without any sacrifice. Proper selection of patients with good indications and effective use of advanced techniques are necessary for TELD to provide the means for a consistent successful resolution. Regarding the appropriate patient selection for TELD, it is important to consider clinical and radiological findings related to prognostic factors and technical aspects including the accessible range of endoscopic forceps. The effective use of advanced techniques to increase the success rate is to approach the target safely and as close to the target as possible. In order to get closer to the target, an accurate targeting process should be performed based on the knowledge of the anatomical characteristics of the index level and the characteristics of endoscopic equipment and tools. Proper positioning of the working cannula by the effective use of obturator insertion technique and foraminoplasty, and utilization of endoscope levering and rotational movement facilitate close-to-target access. Securing a clear endoscopic field of view, adequate annular releasing, and tips and pearls for complete fragmentectomy are also important for successful TELD. The author hopes that this chapter will contribute to the improvement of a surgeon’s technical competence and the development of spinal endoscopy." @default.
