- W4367483752 abstract "A 75-year-old gentleman was admitted to our ward with a superficial wound infection following a left total knee replacement which he had undergone some six weeks earlier. He was otherwise fit with no other previous medical history of note, and routine bloods were normal except for a mild neutrophilia. He was started on a combination of fusidic acid 500 mg t.d.s. orally and amoxycillin 500 mg t.d.s. orally. Two days later, routine measurement of urea and electrolytes revealed he had developed acute renal failure with a serum creatinine of 509 (NRO-120), urea of 37.5 (NR 2.3-6.9). Urine output remained normal and an ultrasound scan of the kidneys, ureters and bladder showed no evidence of obstruction. Repeat urea and electrolyte estimations over the following three days showed little change. On the sixth day of treatment the patient became clinically jaundiced and liver function test revealed a bilirubin of 146 (NR 2-17) together with raised levels of alkaline transaminases and alkaline phosphatase. The jaundice was attributed to fusidic acid therapy and this was immediately withdrawn. Amoxycillin was continued. Liver and renal function were then closely monitored and both returned to normal over the following nine days. A review of the literature relating to this subject reveals only one previous report of acute renal failure in association with fusidic acid.1 Between 1963 and 1996, however, the Committee on Safety of Medicines was notified of 19 reports of renal failure possibly associated with this drug. In the case of our patient no other causal factors were apparent, and we believe that in this instance acute renal failure may have been associated with the use of fusidic acid. We hope that our experience will be of interest and value to other clinicians.
