- W4367486786 abstract "Co-trimoxazole is frequently prescribed for a variety of conditions. I recently encountered two patients where its use was associated with balanitis and genital ulceration. A urologist who was consulted said the complication was not rare. Since reports on this complication are infrequent in general medical journals, I thought it would be useful to publish this information. A 65-year-old male took four tablets of Septrin (sulphamethoxazole 400 mg plus trimethoprim 80 mg) over two days for an otitis media. His only other medication was cetirizin 10 mg (antihistamine) once nightly for a week. There were no metabolic abnormalities. On the first day he noticed itching over the glans penis which he recalled had occurred in the past with the same drug. However, he continued with the drug and over two days noticed oedema and bullae progressing to ulceration of the prepuce; the drug was discontinued. A direct smear did not grow any organisms. The lesions cleared in about two weeks but led to phimosis. A second 45-year-old male took the same drug for an upper respiratory tract infection. He was not on any other medication and there were no metabolic abnormalities. The clinical course was similar. A culture was not performed and recovery was uneventful. There are further reports1 on a 45-year-old male with a similar presentation who was given the drug for tonsillitis and had taken the drug a year earlier, and in a 19-year-old who had been exposed to the same drug. The characteristic lesion often starts as erythema and leads to vesicles and bullae. Challenge testing may at times lead to serious consequences like necrosis.1 Postcoital fixed drug eruption in a man sensitive to trimethoprim-sulphamethoxazole whose wife had taken the drug orally has been recorded.2 In a report on 113 patients with fixed drug eruptions,3 trimethoprim-sulphamethoxazole caused the maximum incidence of 36.3%.
