The first group included 28 (67%) patients (mean age 13.1±0.8 years) after mild COVID-19 (without confirmed coronavirus pneumonia), the second group - 14 (33%) patients (mean age 14.5±0.1.2 years) after moderate or severe disease (with confirmed coronavirus pneumonia). A complex of procedures, according to the approved standard, was prescribed for all patients admitted after outpatient and hospital treatment to the pulmonology department of the state children's sanatorium in order to aftercare. The certain follow-up parameters were evaluated: symptoms severity, life quality, respiratory function and respiratory gases, as well as family medical history and alpha-1-antitrypsin-serpin-1 complex.Patients after moderate and severe COVID-19 had initially lower and less dynamic growth of integral life quality index, more torpid follow-up rates of spirometry, pulse oximetry and exhaled gases. Additionally, the higher incidence degree of adverse family medical history associated with respiratory illnesses was established in the group after new coronavirus infection. Moreover, relatively more deficient alpha-1-antitrypsin and more frequent heterozygous polymorphism type of serpin-1 were found in the group after severe new coronavirus infection.The revealed complex of epigenetic and genetic factors may indicate various risk and development phenotypes of both acute and chronic respiratory diseases.Определить особенности влияния санаторно-курортного лечения на детей с разной степенью тяжести постковидного синдрома, а также выявить взаимосвязь степени его тяжести с данными семейного анамнеза и генетическими полиморфизмами по комплексу α-1-антитрипсин-серпина-1.В двухнедельное проспективное когортное наблюдение были включены 42 подростка после новой коронавирусной инфекции (COVID-19). В 1-ю группу вошли 28 (67%) пациентов (средний возраст 13,1±0,8 года), перенесших COVID-19 в легкой форме (без подтвержденного коронавирусной пневмонии), во 2-ю группу — 14 (33%) пациентов (средний возраст 14,5±0,1,2 года), перенесших заболевание в среднетяжелой/тяжелой форме (с подтвержденной коронавирусной пневмонией). Всем пациентам, поступившим после амбулаторного и стационарного лечения на долечивание в пульмонологическое отделение регионального детского санатория, назначали комплекс процедур по утвержденному стандарту. В динамике оценивали совокупность показателей: выраженность симптомов, качество жизни, функцию внешнего дыхания и респираторные газы, а также семейный анамнез и комплекс α-1-антитрипсин-серпина-1.У пациентов, перенесших среднетяжелую/тяжелую форму COVID-19, отмечены исходно более низкий и менее динамичный в приросте интегральный показатель качества жизни, более торпидные в динамике показатели спирометрии, пульсоксиметрии и выдыхаемых газов. Наряду с этим установлена бо́льшая частота встречаемости неблагоприятного в отношении болезней органов дыхания семейного анамнеза в группе с перенесенным COVID-19 более тяжелого течения. Кроме того, в этой группе пациентов были зарегистрированы относительно более дефицитный показатель α-1-антитрипсина и более частый гетерозиготный тип полиморфизма серпина-1.Выявленная совокупность эпигенетических и генетических факторов может свидетельствовать о разных фенотипах риска возникновения и развития как острых, так и хронических респираторных заболеваний." @default.
