- W4372337968 abstract "Welcome to the second JCJP issue of 2023! We are very excited about this issue of the Journal of Cartilage & Joint Preservation (JCJP), the official journal of the International Cartilage Regeneration & Joint Preservation Society (ICRS). Included in this issue are 10 articles from expert cartilage restoration and joint preservation clinicians and researchers. A summary of the included articles is presented below—please look at the full-text articles available online at cartilagejournal.org. In this issue, we exhibit a selection of articles with the central theme of cartilage and joint preservation, including 3 articles that outline osteochondral allograft transplantation (OCA) and the potential factors that may lead to failure, 1 article outlining the potential for using biomarkers for early arthritic detection, and 4 articles that cover OrthoBiologics and their effect on articular cartilage and the meniscus. Lastly, 2 case reports highlight the technique for addressing a radial meniscus tear and the utilization of juvenile osteochondral allograft implantation. OCA remains a popular option for cartilage restoration, particularly for large lesions involving the underlying subchondral bone. In this issue, Dr Mitchell Mologne and colleagues present 2 articles on OCA. In their article entitled, “Trochlear morphology in healthy people: implications for osteochondral allograft transplantation,” Mologne et al use magnetic resonance imaging to assess trochlear morphology in normal patients (without patellofemoral pathologies), in an effort to better understand how to match grafts with recipients. In their article entitled, “Medial femoral condyle width and osteochondritis dissecans: cause or effect and the implications for osteochondral allograft transplantation,” the authors compare the medial femoral condyle anatomy of healthy patients to patients with medial femoral condyle osteochondritis dissecans lesions in an effort to better understand anatomical characteristics in patients with these lesions. Both of these articles help us to better understand the anatomy of healthy knees, in an effort to best match grafts with patients with full-thickness osteochondral defects. In another article, Manjunath et al present a study entitled, “Does the tidemark location matter in osteochondral allograft transplantation? A finite element analysis.” Here, the authors evaluate the impact of graft cartilage thickness on overall contact pressures across the joint, in an effort to determine if subchondral-bony interface mismatch between host and donor has any negative implications. Several articles focusing on the hot topic of orthobiologics are included in this issue of JCJP. First, Brokhman and Galea propose a new platelet-rich plasma preparation method with a prolonged expiration period (up to 9 months) in their article “A novel method for the preparation and frozen storage of growth factors and cytokines obtained from platelet rich plasma.” Next, Shimomura and colleagues present their review of the literature on clinical studies evaluating platelet-rich plasma and cellular therapy in the setting of meniscus repair in their article “Utilization of orthobiologic augmentation for meniscal repairs: current concepts and future perspectives.” In a separate article, Conte and colleagues assess the utilization of biologics as a stand-alone treatment for degenerative meniscus tears (different from the majority of articles, which focus on meniscus repair) in their article “Orthobiologic injections for treating degenerative meniscus lesions: a matter of facts? Ten years of clinical experience in a systematic review.” The potential implications of using orthobiologics as a stand-alone therapeutic, both in the operating room and in the clinic setting, are fascinating. Moving on from meniscus, in an article by Muran and colleagues entitled, “Effect of increasing hyaluronic acid content in collagen scaffolds on the maintenance of chondrogenic phenotype in chondrocytes and mesenchymal stem cells,” the authors investigate the effects of hyaluronic acid on the chondrogenic phenotype in collagen scaffolds and whether hyaluronic acid promotes the differentiation of mesenchymal stem cells into chondrocytes within collagen scaffolds. Finally, in the article “Serum biomarkers in healthy, injured, and osteoarthritic knees: a critical review,” Burland et al analyzed the current literature for biomarkers present within the knees of healthy patients as well as those with osteoarthritis and those undergoing anterior cruciate ligament reconstruction. Interestingly, they found approximately 164 unique serum biomarkers, with some overlap between healthy and pathologic patients. The final section of this issue of JCJP includes 2 case reports relevant to joint preservation, including 1 on meniscus repair and 1 on cartilage repair. The first article by Vadhera et al entitled, “Transtibial single-tunnel all-inside technique for repair of a complete radial meniscal tear” tackles the technical challenges associated with surgical repair and highlights a single-tunnel transtibial technique for a complete radial tear of the medial meniscus. The next article by Richards et al entitled, “Long-term results of particulated juvenile allograft cartilage implantation: a case report with eleven-year second-look arthroscopy and review of literature,” looks at the long-term follow-up of a patient undergoing particulated juvenile allograft cartilage transplantation, with encouraging outcomes.
