- W4377012779 abstract "Day case surgeries for cardiac implantable electronic device procedures are routinely carried out in health care institutions. Leadless pacemaker implantation is a relatively new technology and not commonly done as a day case procedure. Elective leadless pacemakers are implanted routinely in our institution. In this study we present data on the largest population in the medical literature. To assess whether implantation of MICRA leadless pacemaker as a day case surgery is safe. We retrospectively collected data from all patients undergoing MICRA implantation at our hospital between May 2014 to November 2022 (n=139). Two patient groups were stratified: those had been planned as a day case patient (DC, n=52) and those was planned to admit to the hospital since before the procedure or those received the procedure as a part of their hospital admission (HA,n=87). Complications were classified as 1) Major complications (events resulting in death, permanent loss of device function via mechanical/electrical dysfunction, hospitalization, prolongation of hospitalization by at least 48hours, or system revision), 2) Minor complications if the complications were only observed and did not require clinical intervention. In patients with successfully implanted device (n=138), device function at discharge and at routine first follow-up were evaluated. Stable device function was defined as per the criteria in the Micra Transcatheter Pacing Study in which adequate Pacing Capture Threshold (PCT) was defined as PCT ≤2.0V at a pulse width of 0.24ms and the increase in PCT ≤1.5V from the time of implantation to follow-up visit. There were 58% males (mean age: 56.65year +/- 21.78SD). The indications for pacing were complete heart block(31.65%), AF and pauses(13.66%), infrequent pauses(32.37%). In DC group (n=52), only 8 patients had to stay in the hospital but for less than 48 hours: 2 due to minor groin bleeding, 1 due to patient’s request despite fit to discharge and 5 for observation. Forty four patients(84.61%) were discharged safely on the same day. In 138 out of 139 patients, leadless pacemaker was successfully implanted. The rate of the major complications was 3.85% in DC and 4.60% in HA and that of minor complications was 5.77% and 4.60% respectively. Device parameters at the follow-up were available in 128 patients. Rate of patients at low and stable PCT at follow up were 98% in DC and 97.43% in HA.
