- W4377013394 abstract "PURPOSE: Current fat grafting small animal xenograft models have demonstrated success in human tissue survival. Yet are challenging given the limitations of scale and adipose structure between different species, the grafts’ small size, and the hosts’ immunocompromised status limiting the clinical relevance. There is thus a need for a better large animal translational model capable of sustaining and localizing larger graft sites for direct comparison to human clinical practice. The swine model is a common large animal model of skin and subcutaneous tissues with a record of successful animal-to-human translation. Here, we describe the steps taken to develop a porcine model of fat grafting suitable for testing graft properties and treatment optimization. METHODS: Female Yorkshire Swine received a series of 3cc adipose grafts that were followed longitudinally for 3 months. Adipose processing techniques were evaluated including mincing, cutting, blade usage, and use of mills. Fresh and frozen donor fat were tested to assess allograft sustainability. Additionally, recipient site suture isolation was tested for surgical pocket development. Animals were followed with ultrasound and photography. Terminal grafts were dissected out, weighed then volumetrically assessed with nitrogen gas pycnometry. RESULTS: Porcine adipose does not pass through 16-18G needles prior to processing. Processing using slicing and mincing followed by serial milling with 3.0mm then 1.0 mm plates was sufficient to generate adipose preparation suitable for injection (16-18G cannula). In porcine samples, significant loss of retention (p<0.05) was noted with cryopreservation with significant negative variability of average graft volume (1.28+/-0.16g) vs fresh adipose (1.57+/-0.28) (p=0.028) with total residual fat volume 7.68g in six identified grafts out of eight for cryopreserved vs 12.56g in eight identified grafts out of eight in case of fresh adipose, supporting fresh adipose superiority. Surgical site preparation by suture-delay technique resulted in improved graft localization (P=0.015), qualitative mitigation of spreading in subcutaneous spaces, and overall retention improvement compared to unprepared sites. Histologic analysis of harvested tissues is underway. CONCLUSION: Our data supports the generation of a viable and replicable approach to adipose harvest, processing, and engraftment in the porcine model enabling therapeutic testing on long-term clinically relevant graft volume retention thus amplifying our ability to assess variably sized, timed, treated, and localized fat graft in an immunocompetent animal." @default.
