- W4377015587 abstract "Ventricular arrhythmia (VA) has been successfully ablated from the supra-pulmonary sinus cusps (PSC). However, the electrocardiography (ECG) characteristics for VA originating above the PSCs and the high right ventricular outflow tract (RVOT) are not well-elucidated. We aimed to investigate the ECG characteristics of VA from supra-PSC and high RVOT. Patients with RVOT-type VAs and who also underwent RVOT angiography were included. Three-dimensional electroanatomic mapping was performed in all patients. The successful sites of ablation, which were above the PSCs (supra-PSC group) or within 1 cm below the PSCs (high RVOT group) were confirmed by angiography. VAs from supra-PSC were mapped and ablated by looping the catheter in a reverse U fashion. Thirty-six and 37 patients were identified in supra-PSC and high RVOT groups respectively. In the supra-PSC group, there were significantly shorter intervals measured from the earliest ventricular activation to the peak of the R wave in lead II, III, and aVF on surface ECG during VAs (Table). The cut-off intervals for the earliest ventricular activation to the peak of the R wave were 66ms, 66ms, and 69ms respectively, with high AUC values of 0.944, 0.944, and 0.913 respectively. VAs originating from supra-PSC have shorter intervals from the earliest ventricular activation to the peak of the R wave in II, III, and aVF on surface ECG, which are the characteristics that are well differentiated between VAs originating from high RVOT. These findings help to make the pre-procedure planning to map and ablate the supra-PSC region according to surface ECG." @default.
