- W4377015843 abstract "Loop recorders are being used with increasing frequency for cardiac rhythm monitoring post cardiac ablation. The most common implant technique involves a parasternal incision along the 3rd or 4th intercostal space. When this incision is used in women, the trajectory of the loop monitor and implantation tool can result in partial implantation within breast tissue itself. In this case report, we present an unusual complication of loop recorder implantation in a breast-feeding woman. We aimed to describe a case of failed loop recorder wound healing due to persistent incisional galactorrhea. N/A A 37-year-old woman 9 months post-partum with medical history of supraventricular tachycardia presented with near syncope. She underwent electrophysiology study and was found to have AV nodal reentrant tachycardia which was treated with slow pathway modification. Given concern for continued cardiac arrhythmias, a loop recorder was implanted in the patient’s 3rd intercostal space in a parasternal position angled at 45 degrees in a vector similar to lead II of an ECG. Excellent signal parameters were observed, and the incision was closed with surgical adhesive. Eleven days post implantation, the patient presented with wound dehiscence of the surgical site. Inspection revealed milky discharge from the incision consistent with breast milk (Image below). The patient further noted that when breastfeeding, during milk let-down, she would have increased breast milk drainage from the incision site. The loop recorder was extracted from the open wound. Plastic surgery was consulted, and the wound was closed with a series of layers of absorbable suture. Over several weeks the wound stopped draining and the incision closed. This case highlights a rare but avoidable complication of loop recorder implantation. Consideration must be given to breast feeding status in when implanting women with loop recorders so as to avoid any breast tissue that may result in disruption of underlying milk ducts." @default.
