- W4377016212 abstract "Left bundle branch area (LBBA) pacing has rapidly emerged as a promising modality of physiologic pacing and has demonstrated excellent lead stability and threshold compared to his bundle pacing in multiple studies. To evaluate whether the LBBA pacing modality can allow concomitant AV node (AVN) ablation and same day dismissal in patients with atrial fibrillation who require the ablate-and-pace strategy. 22 Patients (female 61%, male 39%) with average age of 78 ±5 (years old) were admitted for pacemaker (67%)/ICD (33%) implantation and concomitant AV node ablation. Device implantation with LBBA pacing was performed first followed by concomitant AVN ablation through the axillary vein access to allow quicker post-procedure ambulation (figure). The devices were re-interrogated after ambulation and the patients were discharged on the same day after satisfactory post ambulation device check. LBBAP was successful in all these 22 patients, followed by AVN ablation from the left axillary vein access in all but one patient who required the right femoral approach. They all had satisfactory postop chest x-ray, post ambulation device check and were discharged on the same day. After a mean follow up of 6 months, no major complications such as LBBA lead dislodgement requiring lead revision or hospitalization. The LBBA lead pacing parameters immediately post implantation vs 6 moths follow up were: capture threshold 0.8±0.3 [email protected] ms Vs 0.6 ±0.3 [email protected] ms, sensing 9.9±3.9 mV Vs 10.4±4.1 mV, impedance 710±216 ohm Vs 544±110 ohm (p>0.05). QRS duration before and after AVN ablation was 117±32 ms Vs 123±14 ms. Mean ejection fraction before and 6 months after implantation was 44±14 % Vs 46±12% (p>0.05). LBBA pacing not only offers physiologic pacing but also allows concomitant AVNA from a superior approach and safe same day hospital dismissal, in patients with atrial fibrillation who require the AV node ablation and pacing approach." @default.
