Introduction: The early rapid spread of COVID-19 not only significantly impacted access to healthcare for elective, subacute and acute medical conditions across the US, but also resulted in associated worse outcomes in infected patients undergoing procedures. Specific for gastrointestinal surgery, greater mortality odds were detected in patients who developed complications during the pandemic compared to pre-pandemic estimates. Ensuing hospital understaffing also resulted in increased rates of staff burnout, near misses, and other adverse outcomes. The aim of this study was to explore the use and outcomes of inpatients undergoing ERCP for any indication with and without COVID-19 infection. Methods: A retrospective observational study was conducted by querying the National Inpatient Sample for the year 2020. All patients with associated diagnostic ICD-10 codes for COVID-19 were included. Patients undergoing ERCP were identified using respective procedural ICD-10 codes. The primary outcome was the inpatient odds of ERCP in patients with associated COVID-19 infection compared to patients with no COVID-19 infection. Secondary outcomes were inpatient odds of mortality, morbidity and resource utilization comparing these groups, as well as comparing the top indications for ERCP in both cohorts. Multivariate regression analyses were used to adjust for gender, age, insurance status, Charlson Comorbidity Index, income in patient zip code, hospital region, location, size and teaching status. Results: A total of 152,125 patients who underwent ERCP were identified, of which 1,465 (0.96%) had concomitant COVID-19. The mean patient age was 55.7 and 53.9% were female. For the primary outcome, patients undergoing ERCP with COVID-19 had increased odds of mortality (aOR:3.64, p<0.01) compared to patients undergoing ERCP without COVID-19. For secondary outcomes, patients with COVID-19 displayed higher odds of AKI, shock, ICU, ARDS, sepsis, and multi-organ failure compared to patients without COVID-19. In terms of health-care related usage, patients with COVID-19 had longer additional adjusted means for LOS, charges, and costs (Table 1). Conclusion: Patients with COVID-19 undergoing ERCP for any indication displayed significantly higher odds of inpatient mortality, morbidity and resource utilization as compared to patients without COVID-19 undergoing ERCP. Patients with COVID-19 infection had significantly lower associated odds of ERCP compared to patients without COVID-19 infection. While it is not possible to ascertain the specific impact that decreased ERCP odds had on outcomes, these findings highlight the importance of instituting protocols and measures to maximize ERCP access in patients with COVID-19 infection.
