Improved aerobic fitness is positively linked to a slower rate of decline and improved quality of life. The Covid-19 pandemic left a cohort of people with CF reluctant to re-access group exercise environments and lacking the confidence to undertake home exercise. A cohort of adults with CF who could benefit from aerobic exercise but were unable to manage high intensity regimens due to their underlying health status, were identified. From this group, seven participants (4F/3M) were recruited to a twice weekly, six week, virtual, low impact exercise programme, delivered by the CF physiotherapy team. Median age was 54 (33–59 years), median percent predicted FEV127%, with five participants having been referred for lung transplantation. Prior to starting the programme, participants completed a 1-min sit-to-stand (1-STS) test and set individual goals. Baseline observations of HR, SpO2 and BORG were recorded. On completing the six weeks, measures were repeated and participants provided feedback on the programme. All assessments were undertaken remotely. Five participants completed the six week programme with two withdrawing, one due to unrelated injury and the other due to anxiety related to the intervention. Four participants achieved the minimal important difference (MID) of five for the 1-STS (range +2 to +10 STS). The participant who failed to achieve MID had a concurrent exacerbation. Participant feedback was positive. Themes included; enjoying peer exercise, increased motivation and confidence to exercise independently, reduced travel and time burden from the remote nature of telehealth sessions. Provision of a low impact virtual exercise programme for individuals with more severe lung disease was safe, feasible, effective and well received.
