- W4379984369 abstract "<h3>Background</h3> Colorectal cancer (CRC) is one of the most common malignant tumors with high heterogeneity. An array of tumor immune microenvironmental (TIME) characteristics can influence the prognosis and outcome of immunotherapy for cancer. We herein aim to identify intrinsic immunological molecular subtypes (IMSs) of CRC through integrated analysis of transcriptome and genome. <h3>Methods</h3> 587 CRC patients from The Sixth affiliated hospital, Sun-yet Sen University (SYSU cohort) were included for unsupervised clustering analysis of immunological gene expression profiles and comparative analysis of the genome. The Cancer Genome Atlas (TCGA) dataset containing 618 patients was used for validation. <h3>Results</h3> 4 immunological molecular subtypes (IMS1-IMS4) of CRC with distinct molecular characteristics and clinical outcomes were developed and validated in two independent databases. IMS1 and IMS4 were associated with high mutational burden, massive infiltration of immune cells and better survival time compared to the other subtypes. IMS2 and IMS3 exhibited a cold immune microenvironment and correlated with the advanced pathological stage. Moreover, these IMSs presented a distinguished association with immune suppression signature, immunogenic exclusion signature, and immune checkpoints. Finally, the immune landscape revealed the heterogeneity of the microenvironment in patients with different IMSs. (IDDF2023-ABS-0285 Figure 1.
