- W4380357030 abstract "Background: Patients with left ventricular assist devices (LVADs) as a bridge to heart transplantation often face long waiting periods. In order to improve prognosis, stable management with fewer complications is crucial. This study aimed to retrospectively investigate the potential of circulating platelet aggregates as a surrogate marker for risk stratification and early detection of complications after LVAD implantation. Methods: Patients with LVADs who regularly attended outpatient clinics were enrolled in this study. Blood samples were taken, centrifuged, and labeled with a CD61 antibody to detect platelets and platelet aggregates using imaging flow cytometry. The area of the objects captured in images was measured by a custom MATLAB script, and the concentration of platelet aggregates was calculated to evaluate its correlation with clinical factors. Additionally, a prediction model using a convolutional neural network was used for morphological classification of platelet aggregates. The model was trained by the images of platelet aggregates from healthy volunteers in vitro using four agonists, namely, adenosine diphosphate, collagen, thrombin receptor-activating peptide-6 (TRAP-6), and U46619. Results: A total of 47 patients were included in the analysis. The concentration of platelet aggregates was 11.9 ± 10.2% (mean ± SD) and was found to have a significant correlation with the duration of LVAD support and the levels of fibrinogen degradation product and D-dimer (P < 0.05). Patients with infectious complications after LVAD implantation had significantly higher platelet aggregates (P < 0.05). The morphological classification revealed that circulating platelet aggregates from patients with LVADs were mainly categorized as platelet aggregates induced by collagen and TRAP-6. Conclusion: The study successfully demonstrated the ability to detect and classify circulating platelet aggregates in patients with LVADs. A follow-up study is currently under way to examine the use of this method in perioperative LVAD patients." @default.
