• W4381144636 abstract "<h3>Objectives</h3> Patent ductus arteriosus (PDA) is a multifactorial disease with both maternal and neonatal associations. PDA occurs in 1.004 per 1000 live births globally, and 0.25 per 1000 live births in Africa.^1,3 The South African prevalence is 11.9%,² however, this is limited to a single-centered study on VLBW neonates. The aim of this study was to determine the prevalence and trend of PDA, as well as the association between neonatal and maternal variables in the development of a PDA at Charlotte Maxeke Johannesburg Academic Hospital (CMJAH), from January 2013 to December 2020. <h3>Method</h3> The study is a retrospective analysis involving 11,845 neonates admitted to CMJAH from January 2013 to December 2020. Neonatal demographic, respiratory, cardiovascular and infectious variables, as well as maternal chronic conditions, demographics and antenatal care were evaluated for their association with PDA development. <h3>Results</h3> 4.52% of neonates admitted at CMJAH had a PDA. Figure 1 shows the year 2014 as having the highest prevalence which gradually declines but slightly increased again in 2020. The trend was fairly constant and varied at most by 1%. Univariate analysis found 24 statistically significant associations, of which 5 were significant on multivariate analysis. Maternal hypertension (OR 1.87, 95% CI: 1.31–2.67, p 0.001), Gestational age <32 weeks (OR 1.97, 95% CI: 1.38–2.81, p<0.001), Sepsis after day 3 (OR 2.37, 95% CI: 1.81–3.10, p<0.001), Respiratory resuscitation conventional ventilation (OR 2.51, 95% CI: 1.94–3.26, p<0.001). <h3>Conclusion</h3> PDA is a multifactorial disease with a fairly constant and relatively low prevalence. Co-existing congenital heart disease appears to be the most associated variable with PDA development at CMJAH. Other significant variables include respiratory support after initial resuscitation with continuous ventilation, bacterial sepsis after day 3, maternal hypertension and gestational age < 32 weeks. This study provides novel evidence and insight into PDA in the South Africa setting, intended to inform clinicians, so they may effectively employ preventative, health promotive and curative measures. <h3>References</h3> Liu Y, Chen S, Zühlke L, Black GC, Choy MK, Li N, Keavney BD.
